2.15 Transcripts and Enrollment Confirmation Statements
Students may request complete transcripts of their own records or statements certifying their registration at HMS at the Registrar's Office.
HMS Students: Requests for transcripts must be made in writing with an original signature to the HMS Registrar. Signed requests may be:
- faxed to 617.432.0275
- scanned to registar@hms.harvard,edu
